I certify that the information in this application, and in any resume or other materials I submit, is true and complete to the best of my knowledge, and that I personally completed this application. I understand that any false statement, misrepresentation, or omission of material fact may be grounds for rejecting my application or, if I am hired, for discipline up to and including termination, regardless of when it is discovered.

I authorize Single Ply Roofing Experts to contact my references and former employers listed above (except any employer I marked “No” for reference contact) and to verify my work history, education, licenses, and certifications. This authorization does not include criminal history, which will be handled only as permitted by law. I release my references and former employers from liability for providing truthful information in response to these inquiries.

I understand that this application is not a contract of employment. If hired, my employment will be at-will, meaning either I or the Company may end the employment relationship at any time, with or without cause or notice. Only the owner of the Company may change at-will status, and only in a written agreement signed by both of us.

I understand that, if hired, I must provide documents proving my identity and authorization to work in the United States and complete Form I-9 within the time required by federal law.
